Medication Support Checklist

Medication management is one of the most common care challenges for families. Use this checklist to identify gaps before adding care support or moving to a residential setting.

Current medication management

  • Who currently manages medications?
  • Is there a written medication list — name, dose, frequency, prescribing provider?
  • Are doses being taken correctly and at the right times?
  • Are refills being requested before running out?
  • Are expired or discontinued medications removed?

Common gaps to watch for

  • Missed doses due to forgetting, confusion, or difficulty opening bottles
  • Double doses because the person does not remember taking medication
  • Incorrect timing — medications that should be taken with food or at specific times
  • Drug interactions — multiple providers prescribing without full medication list
  • Skipping medication due to cost concerns

Questions for care providers

  • Does this provider administer medications or provide reminders only?
  • What is the policy if a dose is missed?
  • How are medication changes communicated to family?
  • Is a nurse involved in medication oversight, or is it a caregiver?
  • Can medications be locked for safety?
